Registry program

HealthPartners supports provider's health improvement activities by identifying patients who are in need of required services. Care parameters are evidence-based and are defined by practice guidelines established by the Institute for Clinical Systems Improvement (ICSI). We currently have Registries for:

  • Diabetes
  • Chronic Heart Failure
  • Coronary Artery Disease and Cardiovascular Disease
  • Asthma
  • Coronary Obstructive Pulmonary Disease
  • Hypertension
  • Child & Teen Check-up
  • Preventive services

Registries are updated quarterly. Registries can complement your Electronic Medical Record (EMR) data by providing you with data you may not have i.e. diabetic eye exams, dates of hospitalizations and Emergency Department visits, etc.

To access your Registries via the secured Provider Portal:

  • Simply send an e-mail request to Provider E-Commerce Registration. State in your e-mail that you want access to Registry Reporting. Also include: facility name, facility tax ID, your name, phone and your job title. If you have questions about your request for access, call 952-883-7505 and press Option 2 to leave a message.
  • When request has been processed and security clearance obtained, E-Commerce will contact requestor and provide basic instructions for viewing and downloading the registries for your clinic(s).
