Disclosure of Ownership

HealthPartners worked collaboratively with the Minnesota Council of Health Plans and several other Minnesota payers to create a standard form that can be used to submit information related to disclosure of ownership, business transactions and excluded providers. When you complete this form (upon initial contract, when information changes, or upon contract renewal)it only needs to be completed once and then distributed to the healh plans you participate with.

Disclosure of this information is a requirement from the Minnesota Department of Human Services (DHS) and the Centers for Medicare and Medicaid (CMS). They require all health plans, including HealthPartners, to ensure its network providers submit documentation of their ownership, business transactions, and providers excluded from participation in government funded health care.


Compliance & Fraud, Waste & Abuse Training

CMS (Centers for Medicare & Medicaid Services) requires all organizations who provide health care services or administrative services for Medicare-eligible individuals under Medicare Advantage or Part D programs to complete annual Compliance and Fraud, Waste & Abuse Training. Providers who are enrolled in Medicare are deemed to have met the Fraud, Waste & Abuse Training, but still must complete Compliance Training.

On an annual basis, HealthPartners sends an email to each contracted provider's Primary Contact with a link to the training and an Attestation form to be submitted to HealthPartners acknowledging training was completed.

If your organization contracts with other entities to provide health care or administrative services on behalf of Medicare Advantage or Part D members, the training materials need to be provided by your organization. Your organization will be required to attest that an attestation has been obtained from those entities that they have completed the Fraud, Waste and Abuse (if applicable) and Compliance Training.
