ABOUT

Community health needs

Serving St. Croix Valley communities

At Lakeview Hospital and Stillwater Medical Group, our mission is to enhance the health and well-being of our patients and community.

One of the ways we bring this mission to life is to work with community partners to better understand what contributes to and stands in the way of good health. Together we can work to improve health outcomes and well-being across the St. Croix Valley.

As part of our commitment to you, Lakeview Hospital completes a Community Health Needs Assessment (CHNA) every three years.

Our most recent CHNA occurred in 2021. We were part of a comprehensive CHNA that included all HealthPartners hospitals. We worked with the HealthPartners Institute to analyze health data and input from local community members.

Based on the 2021 CHNA, the top priorities and needs in our area are:

  • Mental health and Well being
  • Access to health
  • Access to care
  • Nutrition and physical activity
  • Substance Use

One of our main goals is to make sure everyone has equal access to reliable health care and to work toward a day where every person, regardless of their social circumstances, has the chance to reach their best health.
