We know that health care costs add up quickly. We provide financial assistance so you can pay for the medical care you need.

Lakeview hospital financial assistance

To apply for financial assistance, you must fill out an application form. Application forms must be returned to Lakeview Hospital in-person, by mail or by fax. Patients will need to provide documentation verifying income (such as a federal income tax return) and information about current or potential health insurance coverage (such as Medicaid, Medicare or private insurance).

The federal government requires each hospital to post its standard charges reflecting each inpatient and outpatient service and items provided by the hospital.

Our standard charges may be far different than the amounts you are expected to pay. Lakeview Hospital has negotiated varying discounts with Medicare, Medicaid and most private insurance plans. We can help you figure out how much you’ll have to pay if you call our business office at 1-844-977-2678. They can help you work with your insurance company to understand your actual out-of-pocket costs.
