Serves as a resource and educator to patient, and the Primary Care Team. Coordinates care efforts for a safe, effective, efficient and patient centered care including provider referrals and hospital transition along the health continuum. Provides pro-active disease management using evidence-based practices. We are seeking a full-time clinic RN for our medication refill nurse in our clinic.
The medication refill nurse is responsible for prescription refills of maintenance medications following a physician directed protocol. This position will also do telephone triage for patients calling in wanting same day appointments.

The HUC works under the direction of nursing staff and Utilization Review to assist with insurance authorizations as well as coordinating patient appointments and perform clerical duties. Health Unit Coordinator also act as receptionists and communicates with patients on the phone and in person.
