Serves as a resource and educator to patient, and the Primary Care Team. Coordinates care efforts for a safe, effective, efficient and patient centered care including provider referrals and hospital transition along the health continuum. Provides pro-active disease management using evidence-based practices. Acts as a liaison between patient/family, medical staff and others involved in treatment, promoting patient centered care based on jointly determined health care goals and priorities.

Care Coordination:

Performance is up to standard when:

  • Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans
  • Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care
  • Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention
  • Identifies and secures patient services needed for patient care Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.

Minimum Qualifications/License/Certifications Graduate of an accredited school of nursing Previous nursing experience Current Minnesota License RN Current Healthcare Provider CPR certification Preference given to Certified Care Coordination.

We are seeking a full-time clinic RN for our medication refill nurse in our clinic.

The medication refill nurse is responsible for prescription refills of maintenance medications following a physician directed protocol. This position will also do telephone triage for patients calling in wanting same day appointments.

Olivia Hospital & Clinic is looking for a 64 Hour per pay period Health Unit Coordinator (HUC).

The HUC works under the direction of nursing staff and Utilization Review to assist with insurance authorizations as well as coordinating patient appointments and perform clerical duties. Health Unit Coordinator also act as receptionists and communicates with patients on the phone and in person.

Competitive wage with shift differential for evenings and overnight shifts and weekends. Signing Bonus: $1000

Olivia Hospital & Clinic is looking for a HR professional to join the leadership team providing leadership and management of Human Resources, Education and internal Health & Well-Being.

This position plays an integral role in meeting the organizations Mission, Vision and Values.

The key responsibility for this position is to serve as a resource and guide within the organization for Human Resource practices, policies and employee relations in order to create the optimal employee experience.

The HR Manager will partner closely with all leadership and senior leaders and reports to the Regional Human Resources Director.

Other responsibilities of the HR Manager include administration and interpretation of organizational policies, recruitment and ongoing development of talent, administration of benefits and compensation, payroll functions, leadership and employee advisement, retention strategies, and understanding and supporting laws, regulations and requirements.

The qualified candidate will have a Bachelor’s degree or certification in Human Resources or Management or related degree, and at least five years human resource experience. Previous experience in leadership preferred. In addition, the HR Manager will have sound decision making skills, excellent communication skills, the ability to multi-task and prioritize work, and the ability to empower and inspire at all levels.

Olivia Hospital & Clinic is currently recruiting for an accountant. The Accountant performs general accounting, financial reporting, budgeting and auditing tasks within the Accounting Department.

The qualified candidate will have a Bachelor’s degree in accounting with 3 plus years of accounting experience, Financial Analytic reporting experience, with advanced computer skills in MS Office and accounting software.

Olivia Hospital & Clinic has Medical Laboratory Technician (MLT) or Medical Lab Science Tech (MLS) position available. The MLT or MLS will perform and report tests of all complexity levels, as defined by CLIA on patients of all ages.

Position requires high level of customer service skills to establish and enhance positive relationships with patients, coworkers and others. MLT or MLS for Olivia Hospital & Clinic includes day and evening shifts with call rotation.
