MSHO is for people who are dually eligible for both Medical Assistance (Medicaid) and Medicare Parts A and B. MSHO combines these two programs’ coverages into one plan. That can make it easier to manage your health insurance benefits. With an MSHO plan, you’ll only have one ID card in your pocket and one number to call.

Our MSHO plan combines Medical Assistance with Medicare so you can get all your coverage together. This includes:

A care coordinator will contact you shortly after your HealthPartners MSHO plan begins. In addition, you’ll get your care coordinator’s direct phone number if you have questions or need help. Our Member Services team can also connect you with your care coordinator.

You’ll likely be able to keep the doctors and specialists you already have.

Our MSHO network covers most health care providers in our service area. This includes HealthPartners and Park Nicollet hospitals and clinics, plus many others. You can search our MSHO network to see if your doctors and specialists are included.

You can also search our plan’s drug list (formulary) to see if your prescriptions are covered.

Remember, you won’t pay anything when you get:

MSHO can help you save money. You won’t pay any monthly premium. When you see an in-network doctor or provider, you’ll pay a $0 copay for covered services. If you need out-of-network emergency care, urgent care or renal dialysis within the U.S., you’ll also pay a $0 copay. And you’ll have a $0 copay for all covered prescription drugs.

Keep in mind that you’re still responsible for paying your Medicare Part B premium. There are several programs that can help with Medicare costs.

There are several chances to sign up for or change to a different MSHO plan:

• Once each month
• When you become eligible for both Medical Assistance and Medicare Parts A and B
• During annual open enrollment (October 15 through December 7)
• During a special enrollment period if your eligibility changes
