MSHO: A Minnesota plan for seniors with Medical Assistance (Medicaid) and Medicare

Our HealthPartners Minnesota Senior Health Options (MSHO) plan combines Medical Assistance and Medicare into one simple, affordable $0 plan. You won’t pay any monthly premium. You’ll also have access to a care coordinator. And there are valuable extra perks, too. It all comes together to helps you live more safely, healthy and independently.

MSHO eligibility

You can enroll in our MSHO plan if you’re at least 65 years old and you:

  • Get Medical Assistance without a medical spenddown, and
  • Have Medicare Parts A and B, and
  • Live in one of these Minnesota counties: Anoka, Benton, Carver, Chisago, Dakota, Hennepin, Ramsey, Scott, Sherburne, Stearns, Washington or Wright

With MSHO, you can save money. You won’t pay any monthly premium. You’ll pay $0 copays for covered medical and dental services if you go to an in-network provider. And you’ll have a $0 copay for covered prescription drugs.

HealthPartners MSHO plan members also get a dedicated care coordinator. Your care coordinator helps with your well-being. They also navigate Medicare and Medical Assistance with you. They’ll partner with you to set health goals and manage appointments. And they can help ensure your care providers are in contact with each other.

You’ll get coverage for physical exams and doctor visits. You’ll also have coverage for prescription drugs, emergencies and more. You’ll get access to extra perks like Virtuwell and SilverSneakers®. And there’s even an over-the-counter (OTC) allowance and rewards for completing wellness activities.

We support all the health needs of our members. We do this through HealthPartners programs and resources that boost your well-being. If you need help, our Minnesota-based Member Services staff can assist. With a Medicare Star rating of 4 out of 5, we're proud to be rated above average.

From Oct. 1 through March 31, we take calls 8 a.m. to 8 p.m. CT, seven days a week. From April 1 through Sept. 30, we take calls 8 a.m. to 8 p.m. CT, Monday – Friday.
