Dedicated help from a care coordinator

All MSHO plans come with a care coordinator just for you. Your care coordinator helps you with your well-being. They’ll partner with you to manage your appointments and set health goals. They can help navigate Medicare and Medical Assistance with you. And they can help ensure your care providers are in contact with each other.

Medical, dental and prescription drug coverage – together with MSHO

Our HealthPartners MSHO plan includes all-in-one coverage for common health care:

  • Doctor and specialist visits, including routine physical exams
  • Urgent care and emergency care throughout the U.S.
  • Prescription drugs and prescribed over-the-counter drugs
  • Dental care
  • Medical supplies (durable medical equipment)
  • Hearing exams and hearing aids
  • Eye exams and glasses
  • Long-term care services, like nursing home care
  • Home- and community-based services, if you qualify
  • Personal care assistance, if you qualify
  • And much more

In general, you must get care from in-network providers. And you must get your prescriptions at an in-network pharmacy. But in the U.S., you’ll be covered if you need out-of-network emergency care. You’ll also be covered in the U.S. for out-of-network urgent care or renal dialysis.

$300 over-the-counter (OTC) benefit allowance

Your HealthPartners MSHO plan includes an annual OTC allowance of $300. Each quarter (every three months), you’ll get $75 that you can use to buy health, dental and well-being items not already covered by your prescription drug benefits. You can only use your OTC allowance through NationsBenefits®. And benefit dollars can only be used during each quarter that you get them.

Rewards for completing health screenings

You can earn up to a $100 reward for completing certain key health screenings. These include your Medicare Annual Wellness Visit and bone density tests. They also include breast cancer screenings (mammograms), colorectal cancer screenings and A1c screenings.

More extras for those who qualify

The benefits below are a part of special supplemental benefits for members with certain chronic conditions such as heart failure, dementia, diabetes, cancer or a disabling mental health condition.
