Tablet devices for seniors on both Medicare and Medical Assistance (Medicaid)

Available to eligible HealthPartners Minnesota Senior Health Options (MSHO) (HMO SNP) plan members

Our HealthPartners MSHO plan features valuable extra perks and benefits to help you reach your health goals. If you’re living with certain chronic conditions, you could be eligible for a tablet device to help you live more safely, healthy and independently. This tablet can also make it easier to communicate with your care team. Plus, it’s available at no additional cost to you.

Your tablet helps you manage your chronic conditions

Eligible MSHO members can get a tablet device. This tablet is provided to you at no added cost. The custom tablet is loaded with:

  • Information to help you understand, treat and live with your health condition
  • Health education, engagement and wellness applications
  • The HealthPartners app, so it’s simpler to manage your insurance, or engage with your HealthPartners or Park Nicollet doctor and care team

Tablet eligibility requirements

This benefit is part of special supplemental benefits for members with certain chronic conditions: diabetes, heart disease, depression or cognitive impairment. Not all members will qualify, and all applicable eligibility requirements must be met.

Tablets are limited to one per member every two years.
