How to enroll in our MSHO (HMO SNP) plan

Get ready to enroll in MSHO

Minnesota Senior Health Options (MSHO) plans combine Medical Assistance and Medicare into one.

To enroll in a HealthPartners MSHO plan, you must be at least 65 years old and:

  • Be a United States citizen or lawfully present in the United States
  • Get Medical Assistance without a medical spenddown – If you don’t have Medical Assistance yet, visit the Minnesota Department of Human Services .
  • Have Medicare Parts A and B – If you were getting Social Security benefits when you turned 65, you were automatically enrolled in Medicare. If you don’t have Medicare yet, you can enroll online .
  • Live in our MSHO service area – Plans are available in these Minnesota counties: Anoka, Benton, Carver, Chisago, Dakota, Hennepin, Ramsey, Scott, Sherburne, Stearns, Washington and Wright.

From Oct. 1 through March 31, we take calls 8 a.m. to 8 p.m. CT, seven days a week. From April 1 through Sept. 30, we take calls 8 a.m. to 8 p.m. CT, Monday – Friday.
