Dedicated help from a care coordinator

MSHO plans come with a care coordinator. Your care coordinator helps you with your well-being. They’ll partner with you to manage your appointments and set health goals. They can help navigate Medicare and Medical Assistance with you. And they can help ensure your care providers are in contact with each other.

Medical, dental and prescription drug coverage – together with MSHO

Our HealthPartners MSHO plan includes all-in-one health care coverage:

  • Doctor and specialist visits, including routine physical exams
  • Urgent care and emergency care throughout the U.S.
  • Prescription drugs and prescribed over-the-counter drugs
  • Dental care
  • Medical supplies (durable medical equipment)
  • Hearing exams and hearing aids
  • Eye exams and glasses
  • Long-term care services, like nursing home care
  • Home- and community-based services, if you qualify
  • Personal care assistance, if you qualify
  • And much more

In general, you must get care from in-network providers. And you must get your prescriptions at an in-network pharmacy. But in the U.S., you’ll be covered if you need out-of-network emergency care. You’ll also be covered in the U.S. for out-of-network urgent care or renal dialysis.

Our MSHO plan makes it easier to live healthy. All extras are included in your MSHO plan at no cost.

$300 over-the-counter (OTC) benefit allowance

Your HealthPartners MSHO plan includes an annual OTC allowance of $300. Every three months, you’ll get $75 you can use to buy health, dental and well-being items. You can use your OTC allowance through NationsBenefits®. Benefit dollars can only be used in the quarter you get them.

Perks and benefits to support your well-being

Rewards for completing health screenings

Earn a $100 reward for certain health screenings. These include your Welcome to Medicare Visit, Medicare Annual Wellness Visit and bone density tests. They also include breast cancer screenings (mammograms), colorectal cancer screenings and A1c screenings.

More extras for those who qualify

The benefits below are a part of special supplemental benefits for members with certain chronic conditions such as heart failure, dementia, diabetes, cancer or a disabling mental health condition. Not all members will qualify, and all applicable eligibility requirements must be met. For details and a complete list of eligible conditions, please contact us.

  • A monthly allowance for fresh produce at participating grocery stores
  • A lifelike support pet, with your choice of dog, cat or bird
  • A senior-friendly GrandPad tablet to support connection and wellness
  • Support for caregivers, including coaching, counseling, short-term respite care and psychotherapy, plus transportation support for these services
