Medicare meal and produce delivery benefits for seniors

Available to eligible HealthPartners Medicare Advantage and MSHO (HMO SNP) plan members

Good nutrition is important for better health. Many HealthPartners Medicare Advantage plans and our Minnesota Senior Health Options (MSHO) plan make it easier for our members to get the healthy food they need.

Qualifying HealthPartners Medicare Advantage members: Home-delivered meals from Mom’s Meals®

Many of our Medicare Advantage plans include the HealthPartners Choice Card. The Choice Card is our version of a Medicare flex card. After an inpatient hospital stay or a skilled nursing facility (SNF) stay, you can use your Choice Card for meals from Mom’s Meals. These fully prepared, home-delivered meals can make the transition home easier after you’ve been away.

Qualifying HealthPartners MSHO members: Fresh produce allowance

Fresh food can help people with chronic conditions enjoy a healthier diet. Eligible HealthPartners MSHO plan members can get a monthly voucher-based food benefit allowance of $50 through our Fruits and Veggies Rx program. You can spend your allowance on fresh produce at participating grocery stores. You’ll choose the fruits and vegetables you’d like to get.

MSHO fresh produce allowance eligibility requirements

The fresh produce allowance benefit is part of special supplemental benefits for members with certain chronic conditions, such as heart failure, dementia, diabetes, cancer or a disabling mental health condition. HealthPartners can also help you find food and nutrition assistance. Through our partnership with Unite Us, you can search support programs near you, including:
