Available in northeast Wisconsin, the Birch plan is mindful of both your wallet and your health.
Enjoy the same in-network and out-of-network cost sharing at any provider who accepts Medicare and is willing to see you. You’ll also get a $215 HealthPartners Choice Card to spend on certain non-Medicare-covered items and services. Plus, there’s a $1,000 dental allowance for certain preventive, restorative and periodontic care, like routine exams, cleanings, fillings and gum disease care. Part D prescription drug coverage is included in your plan too.

You can also see out-of-network providers who accept Medicare and are willing to see you – you’ll pay the same costs as if they were in network.

- Out-of-pocket maximum: $5,000 in-network / $10,000 combined in- and out-of-network
- $215 HealthPartners Choice Card you can use toward certain chiropractic services, certain prescription eyewear and more
