With our HealthPartners® Birch (PPO) plan, you’ll find the reliable medical and prescription drug coverage you’d expect but with an emphasis on lower member cost sharing and extra supplemental benefits. Available in northeast Wisconsin, the Birch plan is mindful of both your wallet and your health.

Enjoy the same in-network and out-of-network cost sharing at any provider who accepts Medicare and is willing to see you. You’ll also get a $215 HealthPartners Choice Card to spend on certain non-Medicare-covered items and services. Plus, there’s a $1,000 dental allowance for certain preventive, restorative and periodontic care, like routine exams, cleanings, fillings and gum disease care. Part D prescription drug coverage is included in your plan too.

It’s all available for a $0 monthly premium.

You can also see out-of-network providers who accept Medicare and are willing to see you – you’ll pay the same costs as if they were in network.

Part D prescription drug coverage is included with your plan.

  • Outpatient care: $65 for diagnostic procedures and tests, 10% coinsurance for lab work, $75 for X-rays, and $300 for MRIs/CTs/PET scans
  • Out-of-pocket maximum: $5,000 in-network / $10,000 combined in- and out-of-network
  • $215 HealthPartners Choice Card you can use toward certain chiropractic services, certain prescription eyewear and more
  • $1,000 allowance for certain preventive, restorative and periodontic care, like routine exams, cleanings, fillings and gum disease care
  • In-network cost sharing for up to nine months of domestic travel
