HealthPartners Choice Card: A prepaid health allowance card for seniors

When you enroll in an eligible HealthPartners Medicare Advantage plan, we’ll automatically send you a Choice Card in the mail.

No. When you receive your Choice Card, your card will be ready to use right away – you don’t need to activate it.

The starting balance of your Choice Card for 2026 depends on which HealthPartners Medicare Advantage plan you’re enrolled in:

After you’ve spent all your card’s funds, hold on to it. Don’t throw it away.

If you remain a member of an eligible HealthPartners Medicare Advantage plan, your current card will be reloaded each year in January. The amount reloaded to your card depends on your plan, and it may vary from year to year. Check your Annual Notice of Changes (ANOC) for more information.

You can use your Choice Card at locations that both accept Mastercard and whose primary business category is either chiropractic services or prescription eyewear. Primary business categories are how businesses classify themselves to credit card companies. If you’re not sure of a location’s primary business category, ask before using your card.

You can also use your Choice Card for purchases through TruHearing for covered hearing aids and through Mom’s Meals for covered home-delivered meals after an inpatient hospital or SNF stay.

Your card could be declined for several reasons, including if:

• The total cost of the transaction is more than the funds available on your card
• The location doesn’t accept Mastercard
• The location’s primary business category isn’t for chiropractic services or prescription eyewear
• You’re using your card for hearing aids somewhere other than TruHearing, or for meals somewhere other than Mom’s Meals

If your card is declined and you think it shouldn’t have been, you may need to pay for your service in another way. Then, submit a claim through your HealthPartners account. We’ll review your transaction and, if applicable, reimburse you with funds from your Choice Card.

Your card can only be processed if the transaction amount is equal to or less than your card’s available balance.

If the transaction amount is higher, tell your provider you’d like to pay with two methods:

• First, use your Choice Card to pay for an amount equal to or less than your card’s available balance
• Then, use another payment method to pay for what’s left

Keep in mind that you’ll pay out of pocket for any amount above your card’s available balance.

No. The amount on your card is valid only for services received on or after your plan’s effective date through December 31 (or the date you disenroll from your plan). That means after your plan ends each year, any Choice Card funds you have left over for that year will be lost.

Remember: You can only use the amount reloaded to your card each year to pay for eligible expenses from the same year. The reloaded amount can’t be used to pay for expenses from previous years, and it also can’t be used to pay in advance for expenses in future years.

Contact Member Services. We’ll deactivate your old Choice Card and send you a new one at no cost within seven to 10 days.

Yes. To be eligible for reimbursement with funds from your Choice Card, your purchase must have:

• Been for an eligible expense, and
• Been made between your plan’s effective date and December 31 (or the date you disenroll from your plan)

We must receive your reimbursement request by March 31 of the year following your purchase.

Use your Choice Card like you would any other credit card or debit card. Providers often take payments in person, over the phone or online.

No, your Choice Card can’t be used for OTC purchases. But some of our Minnesota Medicare Advantage plans do include a separate OTC allowance.
