Application information

Regions Hospital uses the services of HealthPartners Credentialing Services Bureau (CSB). CSB staff process applications and conduct primary source verification of applicant’s credentials including education, training, work history and licensure.

Our goal is to complete new applications within 90 days following receipt of a completed application. If you have a question or concern about the status of your application, please contact the CSB at 952-883-5755 or Medical Staff Services at 651-254-9415.

Physician – initial appointment $250
Physician – reappointment $150
AHP – initial appointment $250
AHP – reappointment $150

Fees apply to any practitioner applying for privileges. Please send a check payable to Regions Hospital to the CSB address below. (Applications will not be processed until payment is received.)

Checklist

The Delineation of Privileges (DOP) form should be submitted along with your credentialing application and all other required documents.

  1. Print and complete the Delineation of Privileges for your specialty.
  2. Complete your credentialing application and Regions specific documents and mail with the application fee (see Fee schedule above) to CSB using these instructions:
