Complete the application and provide the necessary income verification. This may include copies of your most recent federal tax returns and/or other verifications of household income. If you’re currently receiving disability income, provide a copy of your most recent award sheet as well as a copy of your most recent tax return. The financial assistance application must be completed in its entirety to process the application.
Application forms must be returned to Regions Hospital in person, by mail or by fax. Our address and fax number are included on the application for your convenience. When your application and documents arrive, they will be reviewed.
Assistance is based on federal poverty guidelines, size of family, household income and expenses, etc. We expect our patients to pursue all sources of payment for expenses before help can be granted.

Our hospital-based clinics may charge a separate facility fee depending on your health insurance. This can result in a higher out-of-pocket expense. Knowing this ahead of time can help you plan for your care expenses.
