Description: The medication management meeting experience survey gathers valuable feedback to improve future service at HealthPartners and Park Nicollet pharmacies. Selected patients that have had a medication management visit with a pharmacist are mailed a one page survey to complete and return in a prepaid envelope. Feedback given in the survey will be used for the improvement of this program.
